I wrote this as a training module for teaching nursing assistants the importance of nutrition and hydration in caring for the elderly. I included both general nutrition knowledge and special considerations for the elderly population. I really think those caring for the elderly should be specifically trained in how nutritional status can really affect that population.

Nutrition Assessment and History Form

For long term care, or where I see a client multiple times, I like having a form where I can put all the relevant information about the person and changes/needs they have in one place. This form I adapted to meet my needs from another one someone had given me. Feel free to make copies and use it, or adapt it for your needs.

Low Sodium Diet Education


Nutrition education, should be so easy, but is it? How much information is enough? How much information is too much? If someone has been eating a certain way for the past 40 years are they really going to decide not to because we hand them a 10 page handout on foods they should avoid? Are they even going to read it? Personally I don't think so. I think the most effective nutrition education is one that keeps things simple, one page, a few big offenders they can avoid and just the basics. Here's a low sodium diet education handout I use, feel free to copy it to help you with your nutrition education, I just ask that you don't remove my name from it, plagiarize it or sell it for profit.
